Iboga Experiences

People looking for first-hand accounts of iboga find them in three places: Reddit, the Erowid archive, and clinic testimonials. None of the three is representative, and the ways they are unrepresentative differ.

This section is about reading them, not replacing them. Accounts are the only source for what the experience is like from inside, and the clinical literature is the only source for what it looks like from outside. Both are needed and neither is sufficient.

What is actually measured

The largest quantitative study asked 499 people what had happened to them, most of them on site and the rest within a day or two of leaving. Seven dimensions came out of it: narrative and symbolic visions, visual changes, discomfort and challenge, cosmic and archetypal visions, introspection and personal transformation, somatosensory hypersensitivity and physiological activation, and dissociation.

The same disclosure we ask of every other source belongs here. The 499 were clients of a single for-profit clinic in Tijuana, recruited as they came, and two of the paper’s authors are employed by that clinic. Four in five were men, nearly nine in ten were born in the United States, and three quarters had come for neuropsychiatric reasons rather than for addiction. It is the largest measurement that exists. It measures one clinic’s clientele.

On timing, the same study found two thirds put their most intense effects between the second and fourth hour, and gave a mean total duration of about nineteen hours. Individual answers ran from nothing to eighty-five hours, a spread the paper reports alongside a standard deviation too small to be consistent with it.

In a hospital series of fourteen people, the subjective experience lasted three to seven hours, and clinicians observed something the accounts rarely dwell on: severe ataxia, with every patient unable to walk without support and needing a nurse to reach the bathroom. It peaked two to six hours after the dose. Most had recovered by twenty-four hours; five of the fourteen had not, and were clear a day after that.

The timings everyone quotes have a shaky origin

Almost every description of the ibogaine experience uses a three-phase scheme: an acute phase of four to eight hours, an evaluative phase of eight to twenty, and a residual phase beginning twelve to twenty-four hours in and lasting a day to three days or longer.

That scheme comes from a 2001 review whose author was explicit about how he built it, describing it as a composite derived from interviews with patients and treatment guides together with general descriptions in the literature. It was never an observational study.

Why two people on the same dose do not have the same night

The range in that 499-person study is the thing to hold on to: from under an hour to eighty-five. Some of that is what people mean by the question. Some of it is pharmacology.

Ibogaine is converted to noribogaine by a liver enzyme, CYP2D6, whose activity varies widely between people on genetic grounds, from poor through to ultra-rapid. In a controlled study, twenty-one healthy volunteers were given a 20 mg dose of ibogaine, half of them after six days of a drug that blocks that enzyme. Blocking it roughly doubled exposure to the active compounds, and the authors concluded it would be prudent to genotype people before treatment and at least halve the intended dose in poor metabolisers. That was a chemical stand-in for the genotype, at a dose far below a treatment one, which is the caveat that belongs with the finding.

The same enzyme governs cardiac exposure. The Dutch hospital team reached the same conclusion from the other direction, recommending that any repeat dosing be done only under QT monitoring and on the basis of CYP2D6 genotyping.

Two people given the same milligrams per kilo are not receiving the same dose in any sense that matters, and outside a research setting the genotype is almost never measured. Ibogaine and the heart sets out what that means for risk.

The archives skew, and the archive says so

The people who run Erowid have published, under peer review, an unusually frank account of what their collection is not. Roughly a third of submitted reports are published, each read by two trained volunteers before a senior reviewer decides. Of the reports that state a gender, 84 per cent say male. And the authors name the two biases that follow: reviewers may favour reports about unusual substances, and contributors write about what they themselves find interesting.

The same paper reports something more useful than any tag count. From the drug taken, the dose, the combination, the setting and the writer’s gender, the authors could not predict whether an experience would be recorded as glowing, as difficult, or as tipping into habituation. Their models caught fewer than one in two hundred of the difficult ones. Whatever decides that, it is not legible in the report metadata.

The ibogaine corpus is also thin. In the snapshot behind a 2018 analysis of single-substance reports, the archive held 32 ibogaine reports against 718 for LSD. At that size a handful of accounts moves the picture, which is a reason to read any one of them as an account rather than as evidence of a rate.

Reddit tilts the same way, and less is known about it than people assume. No peer-reviewed study has analysed the ibogaine communities themselves. The paper usually offered as evidence looked at two opioid recovery subreddits between 2018 and 2021, where ibogaine turned out to be the drug discussed most often. Its authors describe negative experiences there as rare, in the discussion rather than the results, from 151 posts and with no denominator.

What the follow-up studies found

They exist, they are small, and most were run by people with a stake in the outcome.

The most-quoted figures come from a retrospective survey of 88 people treated at one clinic in Mexico: thirty per cent never used opioids again, forty-one per cent were abstinent when surveyed, and seventy per cent relapsed at some point. The 88 were drawn from 336 people the clinic had treated, so anyone who had died, was too unwell to answer or could not be found is absent by construction. The authors say as much.

A separate prospective study followed thirty people over twelve months and found improvement greatest at one month, never again reaching that level. By the twelfth month only fourteen of the thirty could be reached at all.

The difficulty people named most often was not the experience. In the same Mexican survey, re-analysed on the seventy-three participants who answered the relevant questions, sixty-one per cent of the described challenges fell after treatment, with integrating what had happened into ordinary life the commonest theme, making up much the same share of what was said by people the study counted as successes and as failures.
